Tyson Medical Clinic Membership Benefits & Terms
Patient Memberships:
A 3 month commitment is required for regular patients (verbal commitment only and membership may be billed monthly). Membership fees will be charged monthly or may be paid quarterly or annually.
Cash Pay
Cash paying members must pay membership fees quarterly. Refills may only be written for months that have been pre-paid.
Scheduling Appointments
Appointments are required for all patients to receive care. Walk-in patients may be fit into schedule only if the schedule allows and are to be de-prioritized to scheduled appointments.
Patients who have never held a membership may schedule a free consultation, but must activate a membership or pay a one-time visit fee of $210 (unless using the Test & Treat Package) in order to receive care.
Patients with past due invoices or past due membership fees must pay all past due balances before receiving care.
Patients who have ended a membership within one year must renew membership and pay a reenrollment fee before receiving care.
Once one year has passed, patients are not liable for the reenrollment fee when reactivating a membership.
Care
Receiving care may refer to receiving:
- Medical advice or recommendations
- Diagnostics, medications, wound care, etc.
- Prescription orders
- Referral to another healthcare provider
- Doctor’s excuse
- Any other form of services, treatments, or interventions to maintain or improve health
Prescription Drug Benefits
Patients must have an active membership at the time of pickup to receive Tyson Medical prescription drug benefits. Benefits are only eligible at Tyson Drug Store pharmacies (Tyson Drugs in Holly Springs, G&M Pharmacy in Oxford, and Potts Camp Pharmacy in Potts Camp).
Price schedules for the prescription drug benefits are as follows:
- Free formulary: limited formulary can be found on website
- Cost + $10: applies to all non-controlled prescriptions not on the free formulary
Tyson Medical drug benefits may apply to any prescription filled at our pharmacies regardless of the prescriber (benefits apply even if Tyson Medical provider did not prescribe the prescription).
Patients with insurance may elect to use their insurance and pay the full copay OR use the Tyson Medical Prescription Plan pricing. Tyson Medical drug benefit cannot be applied to an insurance copay due to pharmacy contract obligations. The pharmacy will assume to use the cheaper of the insurance copay and the Tyson Medical Prescription Plan price when charging members.
Labs
Labs are to be charged to patients prior to placing lab orders unless under unique scenarios. If labs are unable to be charged in advance, patient must attest to paying the full costs of labs as soon as Tyson Medical is able to provide total costs.
Terminations:
Upon termination of membership, all member benefits are revoked and any prescriptions still with refills written by Tyson Medical providers will be discontinued.
Membership Reactivation:
If a membership was cancelled within a year, the patient must pay 3 months of membership fees up front in order to reactivate a membership. If more than one year has passed since last membership, no reactivation fee is required. Membership may only be reactivated after all unpaid invoices or bad debt is paid in full. Upon reactivation of a membership, all prescriptions previously written by Tyson Medical providers that are still in date may be reactivated unless DC’d by prescriber.
